TRT/Hormone Therapy (i.e., Testosterone Injection Therapy) – Intramuscular Injections vs. Subcutaneous Injections

MultiGen Wellness has years of extensive experience in hormone treatment for both male and female patients. From creams and tablets to injections, we have explored a wide range of therapy protocols, measuring their effectiveness across our vast patient base and analyzing the outcomes of all the current therapies available.
We have developed a protocol for men that has proven successful for thousands of patients’ lives. This protocol involves minimal weekly maintenance and relatively painless, easy treatments.
My name is Joshua G. Crampton, and I am the founder of MultiGen Wellness Endocrinology Clinic. Not only have I been a patient of hormone therapy for 14 years, but I am also a certified bodybuilding coach and certified nutritional coach. I understand how the body works and how it uses hormones to build and heal itself.
The Pain of Injecting Testosterone:
I can personally speak to the difficulties of injecting testosterone into my buttocks with a 23-gauge needle, deep into the muscle. I’ve experienced the frustration of hitting a blood vessel, resulting in a spurting stream of blood on the back wall of my bathroom after removing the needle.
I know the pain that comes with intramuscular injections, and the anxiety of wondering if I’ve actually hit the muscle—sometimes sticking myself 2-3 times because I wasn’t sure. I’ve walked around with a swollen, egg-sized bulge in my buttocks for a week after missing the muscle completely.
Let’s face it: injecting yourself or receiving an injection is no fun for anyone, ever.
That’s why, here at MultiGen Wellness, we have extensively studied all known forms of treatment to determine what works best—requiring the least pain, with minimal maintenance.
Treatments:
We’ve found that simple injections in the belly or fat area (subcutaneous) work the best. Testosterone Cypionate, with its long-lasting ester (half-life), is our preferred choice. Our primary goal is to get male patients’ testosterone levels up to 800-900 as quickly and safely as possible. In the early phases of treatment, we recommend multiple shots per week. Once target levels are met, we suggest reducing to weekly maintenance shots, which are larger in size, but the main goal at this point is simply “maintenance” — keeping the testosterone levels high.

MultiGen Wellness follows a few core principles regarding standard male and female hormone therapy protocols. Here is an excerpt from the MultiGen Wellness Philosophy:
Excerpt:
Philosophy #9: Intramuscular injections are dangerous, outdated, unnecessary, and painful. We do not condone them. We support subcutaneous injections, either in the belly or buttock, using small needles.
Philosophy #10: For men, we offer four base medications:

Philosophy #11: For women, we offer three base medications:

Testing and Procedures:
One of the key differentiators between MultiGen Wellness and most local clinics is our comprehensive testing protocols. We are an Endocrinology Clinic, not just a testosterone or hormone therapy clinic. Our focus is on treating the entire endocrine system—whether you’re male or female.
Interestingly, many men initially express hesitation when they contact us. When we inform them that we require testing for over 15 different hormones at the outset, they often respond, “I only want to check my testosterone.” However, once we educate them on how crucial their other hormone levels are to their overall health and well-being, they quickly understand the value of a more thorough approach.
If you have any questions or would like to learn more about our testing and procedures, don’t hesitate to reach out to our team. We’re here to help!
