The Growth Hormone Decline Nobody Talks About
Most people have heard about declining testosterone and estrogen with age — these hormone changes are well-publicized in both medical literature and popular culture. Far less discussed is the parallel decline in growth hormone (GH) and its downstream mediator insulin-like growth factor 1 (IGF-1), collectively called somatopause. Sermorelin acetate is a synthetic analog of the first 29 amino acids of endogenous growth hormone-releasing hormone (GHRH-1-29). GHRH is the hypothalamic signal that instructs the pituitary gland to manufacture and release growth hormone. By mimicking GHRH, sermorelin stimulates the pituitary’s somatotroph cells to produce and secrete GH in the same pulsatile pattern the body uses naturally — primarily during slow-wave sleep.
This is fundamentally different from direct human growth hormone (HGH) injections, which bypass the pituitary and deliver synthetic GH directly into the bloodstream. Sermorelin preserves the normal pituitary-hypothalamic feedback loop, which means:
- GH levels rise physiologically rather than pharmacologically
- The body’s natural regulatory mechanisms prevent GH excess
- IGF-1 levels rise gradually and stably rather than spiking
- Risk of adverse effects associated with GH excess is substantially reduced
- The pituitary gland is stimulated and maintained rather than atrophied

Sermorelin is often combined with ipamorelin (a GH secretagogue that works through a different receptor pathway) to produce a synergistic increase in GH secretion — known as the sermorelin/ipamorelin protocol, one of the most popular combination approaches in anti-aging medicine.
Who Is a Candidate for Sermorelin Therapy?
Adults over 35 experiencing symptoms consistent with GH decline — fatigue, body composition changes, sleep deterioration, reduced recovery, cognitive changes — who have confirmed low or low-normal IGF-1 levels are typically good candidates. Sermorelin is not indicated in active malignancy, pregnancy, or in patients with pituitary pathology.
