Program Description

MultiGen Wellness sends patient care text messages to enrolled patients who have opted in. Messages may include appointment confirmations and reminders, lab and visit updates, medication reminders, secure patient portal sign-in links, account and billing notifications, and two-way messages for care coordination with your care team.

How to Opt In

You opt in by providing your mobile phone number and agreeing to receive text messages when you complete our patient enrollment form, or by texting START to our number. Consent to receive text messages is not a condition of purchase or of receiving care.

Message Frequency

Message frequency varies based on your care needs.

Cost

Message and data rates may apply. Please contact your wireless provider for details about your messaging plan.

How to Opt Out

You can cancel the SMS service at any time by texting STOP to any message you receive from us. After you send STOP, we will send a confirmation message and then stop sending you text messages. You may opt back in at any time by texting START.

2. Minimum Commitment:

Each of our programs requires a minimum 3-month commitment. This allows us to provide you with consistent and effective treatment over a sustained period.

3. Cancellation Policy:

  • After 3 Months: You may cancel your program at any time after the initial 3-month commitment with 30 days’ written notice. No cancellation fees will be assessed beyond the remaining service fees for the 30-day notice period.
  • Before 3 Months: If you choose to cancel your program before the completion of the initial 3-month commitment, the full cost of the 3 months will be considered your cancellation fee. This ensures fair compensation for the resources and services dedicated to your care during that period.

4. Exceptions:

In rare and exceptional circumstances, MultiGen Wellness may consider issuing a partial or full refund based on individual merit. Any such decisions will be made at the sole discretion of MultiGen Wellness management and on a case-by-case basis.
