What Is DHEA?

DHEA (dehydroepiandrosterone) is a steroid hormone produced primarily by the adrenal glands, with smaller amounts produced in the gonads and brain. It is the most abundant steroid in the human bloodstream and serves as a precursor to both testosterone and estrogen — the body converts DHEA into sex hormones as needed.

DHEA-S (DHEA sulfate) is the sulfated, stored form of DHEA that circulates in the blood. It is more stable and longer-lasting than DHEA itself, which is why DHEA-S is the form measured on standard lab panels.

Why DHEA Matters for Hormone Health

DHEA production peaks in the mid-20s and declines steadily with age — by age 70, most adults have DHEA-S levels 70–80% lower than their peak. This decline contributes to:

What Low DHEA-S Looks Like Clinically

Patients with low DHEA-S often present with symptoms that overlap significantly with low testosterone and low estrogen: fatigue, low libido, reduced motivation, difficulty maintaining muscle mass, and cognitive fog. This overlap is not coincidental — low DHEA-S is both a marker of declining adrenal output and a contributor to the downstream sex hormone decline driving those symptoms.

Women with low DHEA-S frequently experience pronounced low libido — because adrenal DHEA is a critical testosterone precursor in women (more so than in men, where testicular production dominates).

DHEA-S and Stress

Chronic stress upregulates cortisol at the expense of DHEA production — both compete for the same adrenal precursor (pregnenolone). This is sometimes called “pregnenolone steal.” High-stress individuals may have adequate cortisol but depleted DHEA-S — explaining why they feel physically functional (cortisol drives alertness) but have low libido, low motivation, and reduced resilience.

How MultiGen Wellness Uses DHEA-S

DHEA-S is included in every MultiGen Wellness intake panel. A low DHEA-S level in the context of low testosterone and high cortisol markers creates a clinical picture that changes the treatment approach — supplementing sex hormones alone without addressing adrenal output may produce incomplete results.

DHEA supplementation is considered for patients with documented deficiency, evaluated in the context of the complete hormonal and metabolic panel, and monitored with follow-up labs.

Interpreting Your DHEA-S Result

Standard reference ranges for DHEA-S are broad and decline with age — they reflect population averages, not optimal levels. A DHEA-S in the “normal” range for a 60-year-old may be significantly below optimal for a 45-year-old with active symptoms. MultiGen Wellness interprets DHEA-S in the context of age, sex, and the complete hormonal picture rather than relying on broad population reference ranges.
