What Is Estrogen Dominance?
Estrogen dominance is a state of relative hormonal imbalance in which estrogen’s effects are not adequately balanced by progesterone — either because estrogen levels are elevated, progesterone is depleted, or both. It is not a single diagnosis with a universal lab marker; rather, it is a clinical pattern characterized by a collection of symptoms that reflect unopposed or excessive estrogen activity.
Estrogen dominance affects both women and men, though through different mechanisms and with different presentations. It is one of the most common hormonal patterns seen in both perimenopausal women and middle-aged men with elevated body fat — and one of the most frequently missed on standard hormone panels that only measure total hormone levels without assessing the relative balance between them.
Causes of Estrogen Dominance in Women
In women, estrogen dominance most commonly arises during perimenopause, when progesterone begins to decline years before estrogen does. The resulting estrogen-progesterone imbalance — even with estrogen at normal levels — produces the classic estrogen dominance picture:
- Heavy, irregular, or prolonged menstrual periods
- Worsening PMS (mood swings, irritability, bloating, breast tenderness)
- Weight gain, particularly around the hips and thighs
- Water retention and bloating throughout the month
- Fibrocystic breast changes
- Uterine fibroids or ovarian cysts
- Fatigue and brain fog
- Anxiety, especially pre-menstrually
- Decreased libido
Other contributors include xenoestrogen exposure (estrogen-mimicking chemicals in plastics, pesticides, and cosmetics), overweight/obesity (adipose tissue produces estrone via aromatase), chronic stress (cortisol competes with progesterone for the same receptor), poor liver function (impairing estrogen clearance), and dysbiosis (altered gut microbiome reducing estrogen excretion).
Causes of Estrogen Dominance in Men
Men produce small amounts of estradiol normally through aromatization of testosterone — this estradiol is essential for bone density, libido, and cardiovascular health. Problems arise when aromatase activity is excessive, producing estradiol levels that outpace testosterone.
The primary driver in men is excess body fat, since adipose tissue is the main aromatase source. Additional contributors include alcohol consumption (which increases aromatase and impairs liver estrogen metabolism), zinc deficiency (zinc inhibits aromatase), certain medications, and aging.
High estradiol in men causes: water retention, mood changes and emotional sensitivity, gynecomastia (breast tissue development), reduced libido, difficulty with erections despite adequate testosterone, fatigue, and increased fat accumulation — particularly around the chest and abdomen.
Addressing Estrogen Dominance
Treatment depends on the underlying cause and clinical picture. For women, micronized bioidentical progesterone supplementation is the most physiologically appropriate intervention — restoring the progesterone-estrogen balance that perimenopause disrupts. For men on TRT with elevated estradiol, aromatase inhibitors (anastrozole or exemestane at low doses) may be appropriate when clinical and laboratory evidence supports their use.
Lifestyle factors — weight reduction, alcohol moderation, reducing xenoestrogen exposure, supporting liver function with cruciferous vegetables and adequate hydration — provide meaningful adjunctive support but rarely suffice alone when clinically significant imbalance exists.
