By using the MultiGen Wellness website, patient portal, or services (“Services”), you agree to these Terms. If you do not agree, do not use the Services.
2. Who We Are
MultiGen Wellness provides telehealth hormone-optimization and wellness care through licensed providers, serving patients across the United States. Phone: 1-800-259-0015.
3. Not for Emergencies
The Services are not for medical emergencies. If you have an emergency, call 911 or go to the nearest emergency room.
4. Eligibility
You must be at least 18 years old and a U.S. resident to use the Services.
5. Telehealth Consent
You consent to receive care via telehealth (phone/video). Telehealth has benefits and limitations, and a provider may determine that in-person care is required.
6. Medical Care & No Guarantee
All care is provided by licensed providers based on their clinical judgment. Prescriptions, including hormone therapy, are issued only when a provider determines it is appropriate after evaluation. We do not guarantee any specific treatment, prescription, or result; individual results vary.
7. Fees & Billing
Consultation fees, medications, and laboratory testing are billed separately and are due at the time of service. You authorize us to charge your provided payment method for the Services you request.
8. Medications
Prescribed medications are dispensed through licensed pharmacies; use them only as directed by your provider.
9. Cancellations
Appointments may be rescheduled with advance notice; missed appointments may forfeit the consultation fee.
10. Refunds
Refunds are governed by our Refund Policy.
11. Privacy
Your information is handled per our Privacy Policy and applicable law.
12. Limitation of Liability
To the maximum extent permitted by law, MultiGen Wellness is not liable for indirect, incidental, or consequential damages arising from the Services.
13. Governing Law
These Terms are governed by the laws of the State of Texas.
14. Changes
We may update these Terms; continued use constitutes acceptance.
