Estrogen dominance is one of the most common — and most commonly missed — hormonal imbalances affecting adults today. What Causes Estrogen Dominance?
The causes of estrogen dominance are multifactorial. In women, declining progesterone production during perimenopause — while estrogen levels remain relatively stable or fluctuate — is a primary driver. Diagnosis begins with a comprehensive hormone panel measuring estradiol, estrone, progesterone, testosterone, SHBG, and the estrogen metabolites 2-OH, 4-OH, and 16-OH estrogens via urine testing. Blood estrogen levels alone can be misleading — the ratio and metabolic breakdown of estrogen are just as important as the absolute level.
Treatment Approaches
Effective treatment addresses both the source of excess estrogen and the impaired clearance mechanisms. Medically, this may involve progesterone supplementation in women to restore the estrogen-progesterone balance, aromatase inhibitors (such as anastrozole) in men undergoing TRT who convert testosterone to estrogen aggressively, and DIM (diindolylmethane) or calcium D-glucarate as nutritional cofactors supporting healthy estrogen metabolism.
