Even “mild” concussive blast exposures can produce subtle but clinically significant pituitary dysfunction that is rarely screened for in standard post-deployment evaluations.
PTSD and chronic combat stress chronically elevate cortisol and dysregulate the HPA axis, directly suppressing testosterone production through the same mechanisms described in our cortisol article. Sleep deprivation — both during deployment and as a chronic symptom of PTSD — further compounds testosterone deficiency. Physical injuries, opioid pain medications (which potently suppress the HPG axis), and the psychological burden of transition to civilian life all contribute to a perfect storm of hormonal disruption.
What the Research Shows
Studies of post-deployment veterans have found testosterone deficiency rates of 20–25% — substantially higher than age-matched civilian males. Veterans with TBI and PTSD show even higher rates of hormonal dysregulation. Despite this, hormone evaluation is not a standard component of post-deployment health assessments in the VA system, and many veterans go years or decades without their hormonal health being properly evaluated.
