You feel off — tired, foggy, not yourself. You get bloodwork. It comes back “normal.” You leave with nothing to act on. This is one of the most common and frustrating experiences in modern medicine, and it usually has a simple explanation: the basic panel a routine checkup runs was never designed to find the root cause of how you feel.

Why “normal” often isn’t the whole story
A standard checkup is built to screen for disease, not to optimize how you function. It tends to run a narrow set of tests and compare each to a wide “reference range” — a range built from the general population, including plenty of people who feel unwell. Fall anywhere inside it and you are labeled normal, even if you are sitting at the very bottom.
Two things follow from this. First, a lot of relevant markers simply never get ordered. Second, the markers that do get ordered are sometimes the late signals — the ones that only move after a problem has been building for years. A more thorough workup does two things differently: it widens the panel, and it reads results in the context of the whole system.
The markers a deeper panel adds
Free testosterone, not just total (and SHBG)
A basic panel often stops at total testosterone — every molecule in circulation, bound and unbound. But most of that testosterone is bound to a protein and biologically inactive. What your body can actually use is free testosterone, the unbound fraction. The link between the two is SHBG (sex hormone–binding globulin), the liver protein that does the binding.
This matters because you can have a “normal” total testosterone while your free testosterone is low, if SHBG is high. Miss SHBG and free T, and you miss the explanation. Low SHBG, meanwhile, often travels with insulin resistance — a clue that points somewhere else entirely.
A full thyroid panel, not just TSH
Most checkups screen the thyroid with a single marker: TSH. But thyroid symptoms — fatigue, weight change, cold intolerance, brain fog — can be present when TSH looks acceptable. A fuller panel adds:
- Free T4 — the main circulating thyroid hormone
- Free T3 — the most metabolically active form
- Thyroid antibodies (TPO, thyroglobulin) — which can flag autoimmune thyroid disease, a common underlying cause
A note on reverse T3: it is popular in functional practice, but its clinical usefulness is genuinely contested and not endorsed by major endocrine guidelines. It is reasonable to treat it as adjunctive information at most, not as something to make decisions on by itself.
Fasting insulin and HOMA-IR
Standard panels check fasting glucose and sometimes HbA1c. Both are real, established markers — but they tend to rise late, only after the body’s compensation has started to fail. By then, insulin resistance may have been developing for years.
Fasting insulin, and the HOMA-IR calculation derived from it, can surface that resistance earlier. This is one of the clearest examples of a marker that is inexpensive, informative, and routinely skipped — even though metabolic health underlies so much of hormone and energy health.
hs-CRP for hidden inflammation
hs-CRP (high-sensitivity C-reactive protein) is a sensitive marker of low-grade, systemic inflammation and is used to refine cardiovascular risk. Chronic inflammation is associated with a range of hormonal and metabolic problems. One honest caveat: an elevated inflammatory marker tells you inflammation is present, not why. It is a reason to look further, not a diagnosis in itself.
Ferritin — the fatigue marker hiding in plain sight
Ferritin measures iron storage, and low iron is a common, very treatable cause of fatigue — especially in menstruating women. It is frequently left off a basic panel. One important nuance: ferritin is also an “acute-phase reactant,” meaning it can rise with inflammation independent of your actual iron stores. That is exactly why it is best read alongside hs-CRP rather than alone.
Vitamin D — worth checking, honest about the evidence
Vitamin D (25-OH) is a genuinely useful marker of a hormone-like nutrient involved in immune, bone, and endocrine signaling, and deficiency is common and correctable. It is worth measuring. Where honesty matters is the specific claim you sometimes see that vitamin D reliably raises testosterone: the evidence there is genuinely mixed. Some studies show a small effect, others show none — particularly in people whose levels are already adequate. Check it and correct a deficiency for good reasons; don’t expect it to be a hormone lever it may not be.
What to do with a wider panel
More data is only useful if it feeds one connected plan. The point of adding these markers is not to collect a longer list of numbers — it is to find the actual root cause so the plan targets the right thing, and then to re-test and confirm it worked. A few principles keep this grounded:
- Context beats any single number. Free T is read with SHBG; ferritin with hs-CRP; glucose with insulin.
- Established markers first. The core of a good workup is well-validated testing; contested tests stay clearly labeled as optional.
- Repletion follows a finding. Correcting a measured deficiency is different from a generic supplement stack.
